Players seeing the athletic trainer or Fairfax Family Practice do not need their baseline information if baselined through Raiders or Fairfax Family Practice.  

All other players should locate your ImPACT Passport Code (emailed to you on the day your child was baselined) to give to the medical provider.  If you cannot locate it, please contact FFPC3 (email ) to get a pdf of your baseline sent to you.
